Deadline: 4:00pm, February 27 (No exceptions)
Office Hours For Submission: IN PERSON ONLY 9:00am – 4:00pm

            To Be Accepted, Your Submission Must Include All Of The Following Items:

☑ Completed Application Form

☑ All Required Supporting Documentation

☑ Certificate Of Liability Insurance

☑ $25.00 Non-Refundable Application Fee

☑ Team Roster

☑ Licenses For All Teams Members      

                    Applications Will Not Be Accepted If Any Of The Above Is Missing.

                                        Incomplete Submissions Will Be Rejected
