In addition to groups and support services on-site at the center, limited “home visits” by the social worker are available to the homebound elderly in effort to meet their goal to “age in place”. The social worker also provides case management services, emotional support and counseling for issues including loss, caregiver’s stress, reduced independence, relationship issues, depression, substance abuse, isolation and more. There is also assistance available to help guide the process of Advanced Directives, Powers of Attorney and organizing of personal affairs. Referrals made to local agencies and resources as needed.
If you need something to be notarized, please call first to schedule an appointment. Notary services are complimentary.
Here are some of the social groups fcailitated by the social worker. Please call Dawn for more information.
Meets to discuss current events and information that interests you. Topics vary and always results in lively conversation. We welcome new members and you can join at any time!
Our group uses literature as a conduit for discussion. Participants share thoughts and feelings about what they have read and relate it (if applicable) to their life experience.
This discussion group centers on a Podcast or TED Talk, which is pre-selected and varies in topic. Our group discusses our thoughts on the topic/s.
Veterans of any age meet monthly to share stories and strive to re-create the camaraderie that is experienced when sharing a common bond.
Ongoing support group. Participants offer mutual support to each other as we share our struggles and grief in a safe non-judgmental venue.
